Linton Forrestall Brooks born August 15 1938 is a former US Ambassador and former Under Secretary of Energy for Nuclear Security and Administrator of the National Nuclear Security Administration NNSA. George Peter "Pete" Nanos is a retired vice admiral in the United States Navy and former director of the Los Alamos National Laboratory. On January 6 2003 Nanos began working as the interim director of Los Alamos National Lab. On July 17 2003 he was named director of the lab. In May 2004 Nanos ordered an emergency shutdown of operations after classified computer disks went missing and a student suffered an eye injury from a powerful laser beam in the same week. His management style made him unpopular with lab scientists. Nanos stepped down as director in May 2005. Glenn Podonsky has led the Office of Health Safety and Security since its creation in 2006. He was director of the Office of Independent Oversight and Performance Assurance OA when it was merged with the Office of Security in 2004 creating the Office of Security and Safety Performance Assurance SSA. Podonsky directed the newly formed SSA until was combined with the Office of Environment Safety and Health in 2006 creating the Office of Health Safety and Security. In 1994 Podonsky was in charge of records retrieval in the search for material relating to dubious radiation experiments conducted during the 1940s. Serial No. 109-66. Material was submitted on test results for Takata Airbag Ruptures and Recalls by the National Highway Traffic Safety Administration. Safety recalls are often marked by tragedy; that is what brings it to our attention. But they are even more troubling when the very equipment being recalled is intended to save lives. Now this morning we will begin piecing together the history of a safety defect that became known only by what appears to us as fits and starts and seemingly has several potential causes. The first known rupture occurred in 2004 in Alabama. Three more ruptures in 2007 led Takata to identify a bad stamp press at a manufacturing facility in Moses Lake Washington. In 2008 Honda recalled 3940 cars in the U.S. however two more airbags ruptured in May and June of 2009 one of which killed the driver. At that point it appears that Takata believed the airbag inflators were being improperly exposed to moisture during the production process. However about that time Takata confirmed that a stamp press was to blame for the at-risk airbags. In early 2011 uncertainty about the cause of the continuing ruptures led to another recall. Previous recalls were expanded in late 2012 upon the discovery that Takata's production records were in disarray. NHTSA demanded that manufacturers broaden the recalls to the national level. On November 8 2014 NHTSA called for a national recall on all vehicles containing driver-side Takata airbags with defective inflators. This defect also affecting millions of passenger-side airbag inflators as well is known to have been the cause of at least seven deaths and an untold number injuries in vehicles made by ten different vehicle manufacturers. Many of these injuries and deaths were caused by shrapnel created due to the explosive rupture of the Takata airbag inflator and associated housing often in low-speed impacts. As of March 2015 recalls covering 11.5 million vehicles had been issued on various Chrysler Ford General Motors Honda Mazda BMW Nissan Mitsubishi Subaru and Toyota vehicles. NHTSA's action came eleven years after the first reported fatality due to Takata airbags after six years of ever-expanding Honda recalls related to the defective Takata airbags and after a failed attempt by the agency to correct the problem using geographic recalls. The first recall on Takata airbags 08V-593 was issued by Honda in 2008 and included 3940 vehicles. Six years and ten recall expansions later Honda acknowledged that as many as six million vehicles contained the deflective inflators. NHTSA's first defect investigation into the matter PE14-016 began on June 11 2014 six days after NHTSA and Takata agreed via conference call to a series of service campaigns that would replace defective airbag inflators only in the "high absolute humidity" states of Puerto Rico Florida Hawaii and the U.S. Virgin Islands. At the time of the agreement Takata informed the agency that all six known inflator rupture events had taken place in Florida and Puerto Rico. U.S. Government Publishing Office paperback
