"Die chirurgische Orthopädie befindet sich in einer Phase stürmischer Entwicklung. Während es früher das Hauptziel der Therapie war durch korrigierende Eingriffe die anatomisch normale Gestalt und damit die Funktion wiederherzustellen kommen in letzter Zeit bei der Behandlung von Gelenkerkrankungen die verschiedenen Methoden der Alloplastik immer mehr in Mode. Dies ist nicht zuletzt darauf zurückzuführen daß die Wiederherstellung der normalen anatomischen Form nur in besonders gelagerten Fällen zur Verbesserung der Funktion und zu einem Schwinden der Beschwerden führt. Demgegenüber wird die Implantation künstlicher Gelenke von verschiedener Seite als die Methode der Wahl angepriesen weil die Augenblickserfolge für den Patienten zumeist spektakulär sind und die ausgefeilte Operationstechnik zudem relativ leicht erlernbar ist. Inzwischen werden allerdings die warnenden Stimmen immer zahlreicher die auf Mißerfolge nach kürzerer oder längerer Zeit hinweisen. Diese bestehen in erster in unerwünschten und unvorhergesehenen Reaktionen zwischen dem Fremdmaterial und den Geweben des Bewegungsapparates. Knochenabbau durch lokale Überbeanspruchung spielt dabei eine wesentliche Rolle. Eine erfolgversprechende Alternative zu der mit mancherlei Problemen behafteten Endoprothesenbehandlung ist die zielstrebige Ausnutzung der natürlichen Fähigkeit der funktionellen Anpassung der Gewebe des Skeletsystems. Das Prinzip dieser Behandlungsmethode ist es durch eine systematische und genau dosierte Änderung der mechanischen Beanspruchung das biomechanische Gleichgewicht zwischen der Größe und Art der mechanischen Beanspruchung auf der einen Seite und der Resistenz und Reaktionsfähigkeit der Gewebe des Stützapparates auf der anderen Seite wiederherzustellen. Damit ergibt sich für die Therapie ein völlig neuer Gesichtspunkt: wo es unmöglich ist an der Ursache der Erkrankung nämlich der Gewebsinsuffizienz unmittelbar anzugreifen da bleibt nur der Weg die mechanische Beanspruchung dieser Insuffizienz anzupassen d.h. erheblich unter das normale Maß herabzusetzen oder durch Änderung der Qualität der Beanspruchung eine Heilung zu erzielen. Hierfür sind eine sorgfältige Planung des Eingriffs und eine spezielle Operationstechnik erforderlich. Mit der beschriebenen Behandlungsmethode lassen sich bei richtiger Indikation Langzeiterfolge bei der operativen Behandlung von drei wichtigen Erkrankungen des Hüftgelenks der Coxa vara congenita Schenkelhalspseudarthrose und Coxarthrose selbst in bisher allgemein als unheilbar angesehenen Fällen erreichen. Der vorliegende Atlas will nicht nur entsprechende Behandlungserfolge demonstrieren sondern vor allem auch dem interessierten Kliniker Richtlinien und genaue Anweisungen für die praktische Durchführung der Eingriffe geben."
