Short narrative descriptions of incidents involving radioactive materials have been separated from the total accident experience because of their special interest to the atomic energy industry. In some instances the 1959-60 accidents have been added to the tables previously published in which were compiled data beginning in 1945. A new table of inadvertent criticality situations is included in this supplement through the courtesy of Mr. William Stratton of the Los Alamos Scientific Laboratory. Also included in this supplement is a tabulation of exposure records at values from zero to 15 rems which reflects a measure of the control of radiation in the work places in Commission operations. The contents include: Serious Accidents 1959-1960; AEC Industrial Injury Frequency Rates Criticality Accidents Radiation Exposure A Summary of Accidents Involving Radioactive Materials in AEC Activities 1959-1960 and A Summary of Accidents involving Fatalities in AEC Activities 1959-1960. A serious accident was defined as an accident required to be reported immediately to Commission Headquarters and included any of the following: a fatalities; b government property damage of $5000 or more; c an external radiation exposure greater than 15 rems received over a short period of time; and d other injury or industrial illness no matter how slight of five or more persons in one accident and other defined accidents. Also includes are descriptions of industrial fatal accidents. A serious accident was defined as an accident required to be reported immediately to Commission Headquarters and included any of the following: a fatalities; b government property damage of $5000 or more; c an external radiation exposure greater than 15 rems received over a short period of time; and d other injury or industrial illness no matter how slight of five or more persons in one accident and other defined accidents. The information in this supplement pertaining to the 1962 experience includes those accidents and incidents designated at "Type A" and "Type B" along with a few others of a lesser degree of importance which may be of interest.
