Tuttavia ancora oggi è spesso considerato uno strumento superato, a cui sono connessi problemi eterogenei: vi è la tendenza ad attribuire al metadone un ruolo nella soluzione o nell'aggravamento di situazioni complesse, di rilevanza sociale o etica; emerge al tempo stesso una visione semplificata del trattamento con metadone, che non valorizza le acquisizioni rispetto alla complessità del trattamento della dipendenza. L'analisi condotta nel volume si fonda sulla consapevolezza dell'efficacia del metadone nel trattamento della dipendenza da eroina e sull'osservazione di come tale strumento non sia usato in modo da valorizzarne le potenzialità.
