Le livre est une suite de dialogues en vers entre diverses personnes "affligées", chacune correspondant à un profil spécifique, et "l'amy sans fard" qui vient leur apporter réconfort et consolation. Ainsi, un "mal marié", un veuf, une veuve, un "riche bourgeois grandement en peine" peu désireux de se marier mais forcé à le faire, un père dont "le fils unique s'est rendu religieux" (renonçant par là à lui donner des petits-enfants), une jeune fille "affligée sur la perte de sa beauté qu'elle a faite par le moyen de la petite vérole", un homme ayant perdu ses biens, une femme riche stérile et d'autres reçoivent la visite de ce mystérieux compagnon. L'amy sans fard suit une tradition remontant à l'antiquité : le poète romain Horace assignait à la poésie la fonction de divertir tout en instruisant. Ici, les vers burlesques communiquent une certaine connaissance, de la part de l'auteur, des profils et des situations de vie abordées tout en s'efforçant d'y apporter une réponse satisfaisante.
